The NMC has a code of professional conduct which all nurses are required to follow.The code states: “As a registered nurse,midwife or health visitor, you are personally accountable for your practice.” The NMC’s Scope of Professional Practice (UKCC, 1992) lays accountability on the individual practitioner. So, if you fail to carry out your tasks competently, you may be called to account for your actions by the NMC – even if the patient concerned does not suffer harm and legal negligence can’t be established. Under no circumstances, therefore, should you undertake a digital rectal examination (DRE) or manual removal of faeces unless you are competent to do so. It is your responsibility to inform your manager if you feel you are not competent. It may be that you need more training and you should make it clear to your manager what steps you need to take to keep yourself competent to perform this work. Assessment of competency for carers or patients should include an understanding of the anatomy of the lower gastro-intestinal tract, the indications, exclusions and contra-indications for DRE and manual removal of faeces, and supervision whilst performing DRE and manual removal of faeces.Assessment should be on an individual patient basis, and on at least three occasions. Training can be assessed through local Continence Advisory Services.A directory of services is available through the Continence Foundation by calling 0845 345 0165.
