Developed with the help of various experts and supported by NAPC, NHS Alliance, NICE and the DH, this pathway supports health and social care teams participating on the QIPP LTC workstream to implement a generic LTC care model and tackle the increasing demand in care needs. It provides different levels of information to support implementation for all the different stages of the patient’s health; Patient pathway, Commissioning, Workforce, Technology, Financial flows and incentives, Data and information. The LTC Commissioning Pathway has been developed to support health and social care teams to implement a generic model in order to manage more effectively LTCs and improve outcomes for patients. The pathway has been informed by teams on the QIPP LTC workstream who are working to implement a system change to the management of LTCs. Appendix 1 provides further detail on the LTC workstream, appendix 2 provides an illustration of the generic LTC model.
